Challenges and Opportunities in the UK Labour Market 1: The Business dictionary defines the labour market as: The nominal market in which workers find paying work, employers find willing workers, and wage rates are determined. Labour markets may be local or national (even international) in their scope and are made up of smaller, interacting labour markets for different qualifications, skills, and geographical locations. They depend on exchange of information between employers and job seekers about wage rates, conditions of employment, level of competition, and job location. Although not as heavily regulated as many other countries, the UK labour markets are regulated in many different ways, these include protections against oppression and discrimination, enhance efficiency, attribute responsibility, improve health and safety conditions, offers security, cut costs relating to employee/employer opportunism, and encouraging behaviour from employees and employers by imposing costs and rewards. Employment regulations exist to provide flexibility to employers and job and financial security to employees. It is vital in reducing poverty, supporting growth and employment with equity. Some argue the UK is not regulated enough e.g. TUC has called for greater protection in some areas of employment and argued for measures to further support collective bargaining. Whereas, Employers have complained that increased regulation restricts flexibility, particularly in times of economic burden. For regulations to be effective they must be implemented well and enforced. Social partner institutions such as, the Arbitration and Conciliation Advisory Service (Acas), the Low Pay Commission (LPC), the Health and Safety Commission (HSC), and the UK Commission for Skills and Employment (UKCES) regulate and influence the labour market via intervention and enforcement, to improve practices within the workplace. Regulation can also exist in the form of accreditation, e.g. a licence is required to perform a particular service. The number of professionals within the UK continues to expand. In 2014 roughly 24% of workers were classified professionals. Regulation has brought an end to the closed shop, restricted the ability to strike, and trade union governance. Although employers complain about regulation, it is recognised that it provides them with a level playing field in the market place. The UK economy is recovering, all be it slower than before. Recovery has not been helped by financial problems within the Eurozone, impacting on export markets. However, employment has now surpassed pre-recession peaks in terms of jobs (2012) and hours worked (2013). More recently there has been an increase in the number of jobs at both the top and the bottom of the job market, with significant fall in skilled trades in the middle resulting in an hourglass labour market. Although long term unemployment has increased, the UK has been relatively successful at keeping people in employment; employment rate, generally increasing each year since 2012, climbing to 73.7% (ONS 2015). Unemployment rates have fallen, however those with low skill levels and the young being more harshly hit. Youth unemployment has fallen sharply, and stands at around 22%. (UKES 2014). Although it should be remembered that substantially more young individuals are staying on in education, and enter the labour market much later than in the past, as graduates. The number of self employed has continued to grow, equating to 83% of net gains in employment since 2007 (Ashworth et al 2014). However, earnings with this sector have also dropped considerably. In addition to the growing self employed, the UK has also seen a substantial increase in other less secure employment. Since 2010 there has been a 50% increase in temporary work, zero our contracts, and government training schemes (OECD 2013). Employment within administrative and secretarial roles, and low skilled manual roles, continue to decline in many industries as technology advances. The world is becoming smaller, and many tasks/roles becoming automated. Many employees will find it necessary to retrain to keep up with technology advances and the changes it inevitably brings. Technology enables change, cost savings, and investment. Employees can now work from anywhere in the country/world quickly and efficiently, with the Financial Services, Specialist Engineering, and I.T. being amongst the industries leading growth and all greatly dependant on technology. There has been an increase of higher skilled workers commanding higher pay, within some industries; these include advanced manufacturing, creative and digital sectors (HM Treasury, 2011). Evidence shows those with higher skills and qualifications are more likely to remain employed; high-lighting the importance of high skills on job market outcomes. Research shows a drop in real wages of roughly 2% each year since 2010. (UKES 2014). By the end of 2013, real wages were roughly at levels of a decade previous. Removal of the default retirement age and changes to pension schemes, has seen individuals working longer, putting pressures on employers to ensure their policies and practices relate to older employees, and older employees needing to constantly retrain to meet the demands of different roles across their working life. There has been a drop of 19,000 people on Government supported training programmes. Public sector employment has also fallen, with numbers at lowest levels since comparable records began in 1999. However there has been a significant increase within private sector. Although it still exists there has been a reduction in the gender pay gap. IFS research shows an average difference in womens pay compared to means is 18%. This increases again after women have children. Freedom of movement within an ever growing EU, in addition to an increase in migration from outside the EU, has brought a large number of migrants into the UK looking for work; the number of non-UK nationals employed in the UK grew from 986,000 to 3.22 million 2015) between September 1997 and 2015. Many of which are employed in low-skilled work. Women, people from minority ethnic groups, people with disabilities, and those aged under 25 and over 55 years are all more likely to be either unemployed or economically inactive (Brewer et al., 2012). More women in the workplace, with more part time roles, and family friendly policies available. 1971 37% climbing to 69% by September 2015. The number of employed has increased to just over 4.1 million between 1992 and 2013; those over-50 increasing by 3.2 million, under-25s falling by almost 800,000. By 2013 the number of over 50s increased to 29% from 21% in 1992. 16 to 64 year-olds with a degree or higher-level qualification increased to +20% in the last 20 years. 2014, approximately 24% of employees were classified as professionals in the UK. Goods and services traded globally, are now five times the value in 1980. 2: Employment relationships are both an economic exchange (agreement to give wages for work) and a power relation (employee agrees to accept the employers authority). The psychological contract (Rousseau 1995), relies upon reciprocal expectations between the employer and employee being met. If violated employees may become de-motivated, and inefficient. Organisations have a huge variety of methods available to them, which involve the employee in the decision making process, making them feel valued and motivated which in turn, supports both the traditional and the psychological contract, enabling a positive working relationship, innovation, and high performing teams, these include: Participation empowers and motivates the employee by involving them in decision making. Teams are also trusted to make decisions for themselves, and encouraged to take responsibility for the quality of their own work. Examples include project groups, whereby employees are delegated resposnisbility to make important decisions, suggestion schemes providing employees with a channel to make comments and put forward new ideas, delegation of responsibility to employees at all levels, particulalry those on the front line and multi directional decision making allowing decisions to feed not only top down, but upwards and sideways too. Employee participation is also sometimes referred to as Employee Involvement. Employee Involvement is the level of employee contribution. A one-on-one approach between employee and management. Employee is involved throughout the decision-making process, therefore encouraging employee ownership. Information is the methods used by organisations to communicate information to their employees e.g. newsletters, information pinned to notice boards, informal networking, emails, and a combination of cascaded briefings, face-to-face communication from senior management and employee representatives. Consultation is the process used by organisations toconsult both directly with their employees, via face-to-face upwards communication, or staff surveys for example, and indirectly via employee representatives. e.g. Joint consultation review issues deemed to be of common interest/ importance to all parties, at unionised/non-union workplaces, predominantly private sector Collective representation Predominantly in public sector and some large private sector businesses. Involves negotiation between employee representatives (unionised/non union) and senior management on pay and other conditions of employment. Partnership schemes employee representatives and employers stress mutual gains. Tackle issues via co-operation. There is a high commitment to sharing information. Employee forums groups of mixed groups unionised/non-union employees meet with managers to share information and consultation. Employees can have a significant influence on the outcome. Partnership working is where employers and employees and/or their representatives work collaboratively to make decisions and plan actions. Partnership can take shape in a formal agreement between an employer and a trade union, but it is also used to refer to a way of working in co-operation (Reilly, 2001). Employee relations today, includes both collective and individual relationships, with alternative direct and indirect channels for employee voice emerging, including communication and involvement via team briefings, staff surveys, project working groups, social media, joint consultation, collective bargaining etc. Although union membership within the UK has fallen since 1980s, and employment relations without the involvement of unions is the norm within some industries, collective bargaining still has a considerable influence on European labour regulations. Typically where an organisation is unionised a significant number of employees will be members. Unions will bargain on behalf of the employee, and/or intervene with disputes between employees and management. As they represent large number of employees they are much harder for management to ignore. They can also protect the employee from victimisation. Another major benefit of belonging to a unionised organisation is that management can be legally bound to take procedures more seriously. In addition, unions have the power to threaten industrial action. Trade unions are still a strong force, primarily in the public sector, and large private sector companies. The majority of working population however, do not have access to union representation. Some parts of the public sector do not recognise trade unions; impacting on collective bargaining, representation in grievance, disciplinary matters etc, within large parts of the private sector. Smaller firms use more informal relations for consultation, using intermediaries offering advice primarily on the internet and in publications. However, many small firms operate without procedures and as such can lead to inconsistencies in employment relations. Non-union consultative committees are becoming common place in private sector, filling part of the gap generated by the decline in collective bargaining; the consultation process giving access to management enabling dialogue. In redundancy situations where no trade unions is recognised, non-union employee representatives must be advised and consulted of redundancies of +20 employees is proposed within a 30 day period. Bryson found that direct voice makes a particular difference in union settings suggesting that more channels makes for an employee voice with more impact. Elgar defines employee voice as the ways and means through which employees attempt to have a say and potentially influence organisational affairs about issues that affect their work and the interests of managers and owners. With a mixture of direct and indirect consultation and communication methods leading to higher levels of commitment, job satisfaction and job discretion. Initially employee voice was firmly equated with trade unions and collective bargaining, but more recently is viewed as a broad range of methods enabling employees to have a say about their organisation; it can be via both formal and informal systems, direct individual channels or indirect collective representation (CIPD 2010). Mechanisms can include one-to-one conversation between employee and employer, email communication, work/project groups, social events, suggestion schemes, profit-sharing, employee consultative committees and trade union representation. Research generally supports the notion that employee voice is beneficial to the working relationship of both the individual and the organisation; where employees are allowed to express their views and opinions, put forward suggestions for improvements, they feel recognised and valued. Data shows employee,employer relationships improve as trust increases and employee engagement and in turn performance increases. (Morrison et al. 2011). Farndale et al. (2011), argues that employee voice enables employees to communicate their views, and creates the belief that their contributions are valued, creating a level of respect and trust for their managers. Employee voice allows employees to feel recognised, listened to and valued, conversely, if no mechanisms for employee voice exist, or employees believe their views will have little or no influence it is likely to affect their attitudes and relationship with management (Farndale et al 2011). Truss et al. (2006) argues that one of the major drivers of employee engagement is employees being able to feed their opinions upwards. Research therefore suggests that there is likely to be a link between employee perceptions of voice and engagement, and increased engagement is proven to increase performance. 3: Organisational misbehaviour is defined as any intentional action made by members of an organisation that defies and violates shared organisational norms and expectations, and/or core societal values, mores and standards of proper conduct. (Yoav Vardi and Yoash Wiener 1996). Misbehaviour can be both minor e.g. poor timekeeping, and major e.g. stealing company resources. It can be internal and external, with the main aim being to hurt/damage others or the organisation, e.g. mistreatment, disruption and theft. Misbehaviour usually occurs where an employee: feels that they have been unfairly treated, unclear of job role bullying and harassment in workplace lack of training lack of equality and choose to repay this perceived unfairness by mibehaving. Misbehaviour inevitably leads to more management controls. Conflict in the workplace can take many forms including: an employee becoming withdrawn from the organisation, two workers who are not getting on, employee representatives and management at stand-off, an employee with a grievance, rivalry between teams, or a lack of trust and co-operation between employees and management. Where conflict exists, an organisation may experience a drop in motivation and in turn a drop in productivity, increased absenteeism, and increased negativity on staff surveys, and a general change in behaviour. By listening to employee views can help identify conflict, and take action to prevent the conflict becoming a problem. If conflict is not managed it can lead to strike action or even tribunal claims; Intervention at an early stage by management is crucial. When negotiation cannot resolve a workplace dispute industrial action typically follows. The three main forms of industrial action are: strike action short of a strike work to rule, overtime ban, or go slow lock-out the employer stops employees from working Official industrial action is authorised by the trade union. Employees have a minimum level of protection. Employees have additional protection where the union has appropriately balloted its members. Unofficial industrial action is action which has not been endorsed by any trade union. There is usually no right to argue unfair dismissal, if dismissed whilst taking part in unofficial action. Some conflicts are extremely visible. e.g., a heated exchange between two employees, or a stand off between management and an employee representative, and may result in misbehaviour. Organisational misbehaviour is defined as any intentional action made by members of an organisation that defies and violates shared organisational norms and expectations, and/or core societal values, mores and standards of proper conduct. (Vardi and Wiener 1996). e.g unauthorised absence, pilfering or sabotage. Conflict that is ignored will frequently escalate Recent industrial action trends are: Increase in private sector strikes however public sector lost significantly more days due to large scale strikes A decline in collective conflict increase in individual conflict, including strike, grievance or disciplinary measures Significant drop in strikes since 1980s mainly due to reduced trade union density and collective bargaining, and partnership working with employers. (WERS) average days lost fell to 0.5 million by 21st century 1999 employment tribunal cases soared +100,000, peaking again in 2006/7 with 132,500 applications Pay is the principal cause of labour disputes with the exception of 2009/2010, principal cause was redundancy The number of ballots up to 650, from 484 in 2013. Increase in multiple claims. Majority relating to equal pay claims against local authorities due to 1997 national agreement to seek single status of pay across employees. It is vital that managers are trained to enable them to deal with conflict. To help managers handle conflict they should: be trained to recognise signs of conflict, handle difficult conversations, manage absence, mediation and negotiation skills speak to their employees and letting them speak freely investigate conflict situations, and give sufficient time to finding resolution encourage open communication and expression of views in meetings and appraisals for example recognise the importance of feelings listen to what their employees are saying identify development and training opportunities explain and include employees in planning treat all employees fairly ensure safe working conditions have clear procedures particularly discipline, grievance, dispute procedures, absence and bullying and harrassment to ensure consistency write mediation into contracts of employment and/or grievance and disciplinary procedures identify when outside help is needed Third party involvement Where a dispute cannot be resolved using negotiation, unions and management may agree to approach Acas to resolve the dispute using conciliation, via independent support and advice. 70% cases who used ACAS, were settled or withdrawn before proceeding to an Employment Tribunal. Conciliation is used when a potential or an actual claim has been made to an employment tribunal. An impartial, independent person supporting two or more people in dispute to resolve their differences. Parties are not required to come face to face, as can be conducted over the phone. Pre-Claim Conciliation(PCC) saves time, money and stress. It promotes a quick solution for the employer and employee which may help to avoid a permanent breakdown in the relationship. Mediation is the most widespread form of conflict resolution. It is a form of early intervention and involves an impartial, independent person, or trained employee supporting two individuals/groups reach resolution, and maintain working relationships. Mediation does not make any judgments or determine the outcomes. They use appropriate tried and tested techniques to get people talking and listening and help determine the underlying causes of the problem. The mediator will try to get the parties to compromise and protect their working relationship for the future. The plus side of this method is, it can improve communication between parties, get them talking again when relationship had broken down, it is without prejudice and is flexible so resolution can be tailored to fit. However, the downside of this method includes, success is determined by how competent the mediator, it may not protect legal rights, and is relient on employee full co-operation and good faith, if pertinent parties are missing the process is weakened. Arbitration is a dispute which is settled by an independent person who considers everyones point of view and then issues a decision which is binding. An employee who believes they have been unfairly dismissed, has a complaint under the flexible working regulations, may have their complaint heard by an independent arbitrator who is appointed by Acas, if both sides agree. The plus side of this process is, it is completely confidential, it is formal and therefore minimises bad faith, it is quicker than the courts as there is less backlog, and the process is much shorter and therefore much less expensive. On the downside, success is dependent upon the arbitrator, right of appeal is limited, and confidentiality is not suitable in all disputes. Government has hi-lighted the importance of mediation in the workplace and extended the use of conciliation in-order to encourage the resolution of workplace disputes. 2012/13, over half of the cases (22,630) referred to PCC were resolved with less than a third progressing to tribunal (Acas, 2013). Should Sex Education Be Offered in Public Schools Sex can be traced back as far as Adam and Eve, the first two people on this planet. Today, all age groups encounter things associated with sex, but it is not a problem that concerns everyone. The problem that has been at hand for more than thirty years is should sex education classes be offered in Public Schools? (1.Teaching Fear; 1996) The reason such debate has arose over the years is because there are many diverse opinions about the topic. Children are now faced with problems at a much earlier age than years passed. There must be a way to reach the children before they are in such need of help and are clueless about the devastating problems around them. Sex education is one step in the right direction, when it comes to the health of our children, and the guarantee of a solid future. Although some people disagree with the idea of sex education classes, in the public school system, the majorities seem to favor it. On the other hand, some people agree with the idea of sex education, but feel like the problem lies more in the way that it is taught, rather than being totally against the idea. Those who are against sex education in class feel as though all the classes would do is create more problems. They are uncertain about the outcome, whether or not it would cause students to be more or less likely to engage in sexual activities and intercourse.( 2.Keen, Cathy. Maggie Hobson Essay

Maggie’s personality in act one gives us the impression that she is very bossy and always wants her own way, â€Å"this is a shop you know, we are not here to let people go without buying†. She is bossing Albert Prosser into buying a pair of boots. I think that this makes her have an unpleasant appearance and is not the sort of person people like very much. During the play the author changes your mind by showing a different aspect to her personality. In act one, she acts very busy â€Å"she crosses and takes her place at desk†, â€Å"she busies herself with an account book†. This makes us think that she is the main boss of the shop. We also think that she does most of the work. The first thoughts of her is that she is anti-social. Alice : â€Å"oh it’s you, I hoped it was father going out†. Maggie : â€Å"it isn’t†. She is acting ‘businesslike’ and doesn’t seem to be acting very friendly to her sister Alice. Maggie is unromantic, she doesn’t think that there is any need for courtship before marriage, â€Å"See that slipper with the fancy buckle on to make it pretty ? Courting’s like that my lass. All glitter and no use to nobody. † This shows that she doesn’t believe in courting. Really courting is nothing like a slipper, but Maggie has a practical personality and doesn’t believe in wasting time. She is a lady who always get what she wants and doesn’t believe that you should waste time over courting when you can get married straight away. Maggie organises the household and arranges the dinner time, â€Å"so that, if you stay more than an hour in the Moonraker’s Inn, you’ll be late for it. † This show that she is trying to take over the main lead in the family role, when it should be her father’s role to say when the dinner should be ready because that was the norm in that era. Maggie has swapped roles with him. Hobson is very rude to his daughters, Alice and Vicky, about their delight in fashionable clothing and the idea of getting them married. Maggie is not included in the conversation of marriage. She then asks him what he thinks of her getting married. He lets her know he hasn’t given her any thought of marriage and is very rude giving his opinion of her, â€Å"but if you want the brutal truth, you’re pass the marrying age, you’re a proper old maid Maggie if ever there was one. † She reacts offended and upset to this remark and tells him that she is only thirty. We can tell that she wants to get married as well as her two sisters, Alice and Vicky.

The Three Religions Essay

The three major religions in the world are Judaism, Islam and Christianity. The largest religion in the world is Christianity followed by Islam. In 33 C.E. Christianity was founded by Jesus Christ. Christianity is more spread around the globe than any other religion. Islam is the second largest religion in the world. In 637-750 Islam was founded by Muhammad. Islam originated in the Middle East, Islam also holds many beliefs and practices that are in common with Christianity and Judaism. Judaism is the twelfth largest religion in the world. Judaism was founded in 586 B.C. The founder of Judaism was either Moses or Abraham. Judaism follow the Ten Commandments like Christianity does. All three religions had and have greatly influenced the whole world. There are many similarities between Christianity, Judaism and Islam. All three of these religions, Christianity, Judaism and Islam are known as â€Å"Abrahamic religions† because of their history to the agreement God made with Abrah am in the Bible. One of the comparisons between Judaism and Islam is they have no priest like Christianity does. Judaism and Christianity also share many similarities, their holy city is Jerusalem and they both follow the Ten Commandments. Islam’s holy city is Mecca and they follow the Sharia Law. Both Christianity and Islam believe the moral teachings of the prophets. They all have spiritual beings like, angles and demons. All three religions are monotheistic which means they believe in one God. Just like similarities there are many differences between the Middle East religions. All three, Christianity, Islam and Judaism have different holy scriptures. Christianity has the bible (Old and New Testaments), Islam has the Qur’an and Judaism has the Torah. Their House of Worship is different. Christianity worships in a church, while Judaism worships in a synagogue and Islam worships in a mosque. Judaism and Christianity’s holy city is Jerusalem but Islam’s holy city is Mecca. Each one of these religions believes in something different, Christianity believes in Jesus while Islam believes in Allah. They follow different rules in their religions such as, Islam following the Five Pillars and Judaism and Christianity following the Ten Commandments. There were also different people who found each religion. Muhammad found Islam, Jesus found Christianity and Moses or Abraham found Judaism. . There were many  similarities and differences between Islam, Judaism and Christianity. Their similarities and differences show how each of the three religions were formed and created throughout the decades. Each one of these religions, Christianity, Judaism and Islam have there own significances to different people and places. Their significances are carried around all over the world. Christianity, Judaism and Islam started in the Middle East but have spread throughout the world. Not only did Christianity, Judaism and Islam spread around the globe but they have impacted the whole world as well.

Health Care Interview Essays Health Care Interview Essay Health Care Interview Essay Certified Nursing Assistant (CNAs) The Certified Nursing Assistant (CNAs) department at Arbor Springs Health and Rehab Center consist of nurses and nursing assistants providing direct, physical, and emotional care to the patients. The type work done in this department deals with one-on-one care from the nursing assistant to the patient. The interviewee has the responsibilities of helping patients with personal care tasks such as feeding, toileting and bathing, as well as transporting patients and providing support to both the patient and his or her family. Nursing assistants also monitor the patients condition, taking vital signs and maintaining or checking medical and personal equipment such as catheters. CNAs assist patients with wheelchairs or other assistive devices and may lead patients in therapeutic exercise or other activities to increase strength and range of motion. The interviewee which is a CNA at Arbor Springs Health and Rehab Center provides the same care to the patients that are under the care of the Rehab Center. The interviewee primary responsibility not only consist of helping the patient with his or her personal needs but is also responsible for general care of the patient, such as ensuring that his or her room is clean. In most facilities, it is the nursing assistants who change linens and assure that the patients immediate area is sanitary. Another very important duty of the CNAS being interviewed is to document all care that they give as a part of the medical record. For example, CNAs record vital signs or urine input/output levels. The interviewee career path for being a CNA at Arbor Springs Health and Rehab Center consist of required training programs which were offered at a technical college. The coursework for CNAs is primarily hands-on and includes training in basic patient care procedures, maintaining hygiene and sanitation, proper handling of bio-hazardous materials, patient safety and medical terminology. In order to be eligible for licensure as a CNA, students must successfully pass a professional board examination after completion of the training program. Training to become a medical assistant or CNA is offered by the Red Cross, community colleges, and online schools and through medical facilities. Some clinics will hire untrained and inexperienced workers and provide on-the-job training, but these opportunities may be hard to come by. Many schools offer training within medical facilities as part of their course programs as well. All CNAs must take an examination before they become qualified nursing assistants. To become a CNA you have to complete a training course which CNAs take anywhere from four to six weeks, and then get licensed in the state where you wish to work. After completing training, you’ll need to take a competency exam that tests both your book knowledge and practical skills that were taught during training. Practice tests and study guides are freely available online, so studying for the test should be a breeze. Moving into a new career after being a CNA will probably take at least two years of education, but the increased pay and benefits you’ll receive upon becoming a registered nurse or even an advanced practice registered nurse will easily make up for the time and money spent on education. There are a few positions that are much similar to CNAs. CNAs, orderlies, psychiatric aides, and medical assistants are often lumped into one large category, and though the positions share some similar duties. Orderlies do not need a degree or certificate, and their interactions with patients are limited to non-treatment activities. Psychiatric Aides is similar to a CNA, except for the work environment. Mental health wards and psychiatric hospitals have different routines than regular hospitals, and psychiatric aides have to be familiar with the unique privacy concerns and treatment needs of mental health patients. A medical assistant does not need a degree or certificate, and their main responsibilities are to help doctors and nurses with routine tasks. Medical assistants have less authority to participate in patient treatment than CNAs. One important attribute for CNAs to be successful is to be professional. Nursing assistants often have to work with hospital patients or nursing home residents who are injured, handicapped or elderly, possibly resulting in feelings of loneliness, self-pity or depression. Its important for nurse assistants to remain professional and calm in the face of this behavior. Nursing assistants must have the ability to put their personal feelings in the background and focus on providing quality patient care under all circumstances, including differences of opinion, race, creed, color, nationality, sexual orientation and religious practices. Another key attribute to being successful as a nursing assistant is to be compassionate. Nurse assistants work daily with individuals unable to care for themselves; their patients are often suffering from an injury, disability or illness. Nurse assistants who treat their patients with compassion will deal with them gently, careful not to cause them additional pain. They will listen and speak to their charges with genuine respect and caring because they know heartfelt kindness CNAs be the first step on the road toward improved health. Although nurse assistants work mostly with patients, they must also have certain characteristics necessary for dealing with co-workers and colleagues. They should be adept at following written and verbal and instruction, they should be able to communicate both orally and in writing, they should feel comfortable working both independently and under supervision and they should consistently collaborate with fellow nurse assistants to provide patients with the best possible care. In conclusion, the interview has some very important role and responsibility as a CNA at Arbor Springs Health and Rehab Center. The CNA work to care for the needs of the patients whether it is direct, emotional, or physical care. The interviewee chosen department is unique because it allows them to meet and care for the needs of their patients. This would include feeding, bathing, or even monitoring and getting vital signs. It also allows the CNAs to build a relationship with his or her patient. As stated by the interviewee, it is not easy being a CNA but I love what I do and I love to see the smile on the face of the patients I care for.

Analysing Product Development Within Dell Computers The Product Development at the Dell Computer Corporation case can be summarized with just its name, product development. The case focuses on development of Dell’s personal computers, highlighting the area of the laptops. The personal computer industry can be dated back to the 1830’s and Charles Babbage with his invention of the first digital computer. However with limitations of materials and marketing his vision mainly stayed on what is called â€Å"the drawing board.† It was not until the time period of World War II when a factory size computer was created by army engineers, they were named Mark I and Colossus and they were 50 feet in size. Through the 1960s and the 1970s only the government, mainly for defense, and big business had the opportunity to use computers. As technology increased and microchips replaced the wires and transistors, and financial availability became friendlier for consumers the microcomputer revolution began. In the 1970s and 1980s Appl e Computer was a successful leader in a commercialized interface that was easy to use. Apple set the technological pace for cramming as much new technology in to their products as possible. IBM who was always trying to play catch up with Apple kicked off its traditional corporate based computer line, with strong direct sales and service. Dell also released its own branded personal computer in 1981. During the 1980s personal computer sales grew from nothing to $40 billion dollars. It was in 1983 when Dell Computer was started by Michael Dell, who at this point in time was a freshman at the University of Texas. He would upgrade IBM compatible computers and go door to door selling them. The success was overwhelming for Dell, and he moved off campus and dropped out of school, with the initial idea of returning back to school if there was failure with the business. However, with $180,000 in sales during his first month the idea of going back to school never entered his mind. The next ste p for Dell was to buy and assemble his own brand name personal computers and get them directly to the customers. This is where Dell’s principle to eliminate the middle man began. With this premise in mind, high growth rates and attractive margins the building of the Dell name began. Soon, Dell would start a 24 hour complaint hotline and they would offer a supply of backup replacement equipment. By 1990, Dell computers had a distinctive line of its own personal computers which won several trade magazine awards for service and products. By 1990 microcomputers accounted for 40% of all computers sold. And there was major competition the brands helping to drive down the costs of manufacturing as well the cost for the consumers. With the development and success of Dell and their direct to consumer sales as well as their laid back sales manner, imitators such as Gateway 2000 and CompuAdd began business. Prostate cancer is the second most common cancer among men, behind skin cancer, and the second leading cause of cancer deaths among men especially in African American men. one out of six men will be diagnosed with prostate cancer during their lifetime. For example, if youre at home having a cookout with 11 of your friends, its likely that two of you at some point in your lives will be diagnosed with prostate cancer. and If youre an African-American man, those odds increase to nearly one in five. Prostate cancer affects African American men twice as often as Caucasian men; the mortality rate among African American is also twice as high. African American men have the highest rate of prostate cancer of any ethic group in the world. In the African American community men are at high risk due to low socioeconomic status, and not getting cancer screening because of no health insurance. With the increased amount of men being diagnosed each year it is shocking that many men still avoid visiting the doctor because it makes them uncomfortable and they are afraid of what the results will be even when some clinics give free prostate cancer screening. While prostate cancer screening test can have abnormal results even when cancer is not present, it is important for men to have prostate screening done.prostate cancer is usually detected through screening, and there are two methods for early detection. The prostate-specific antigen test (PSA) is used, but there are many factors that can inf luence the outcome of the PSA test. Medications such as antihistamines, physical exertion or recent ejaculation can raise a PSA level. Another test for early detection of prostate cancer has been the digital rectal exam (DRE). Although, DRE is a better method for early detection, it is also a hindrance among men, particularly African-American men, to screening and early detection According to American Cancer Society (ACS) and American Urological Association (AUA), the PSA and DRE should be offered annually for all men beginning at age 50. However, they also recommend that African-American men with a family history of prostate cancer should begin testing by age 45. Early intervention is the key to a 5 year survival rate. Prognosis of prostate cancer diagnosed at an early stage is noted as being optimistic. Despite these credible statistics, free prostate cancer screening and early detection remains a problem area for many health care practitioners when it comes to the African American population (Plowden 2009) Madeline Leininger’s Sunrise Model has been used in many studies over the past twenty years. Between 1999 and 2008, over 200 citations of articles were noted using Leininger Theory (McEwen, 2011). She postulated that nurses should attempt to increase knowledge related to the care of people who value their cultural heritage and way of life (McEwen, 2011). The Leininger Sunrise Model of cultural diversity is used to obtain compliance with prostate cancer screening among African-American men; the goal has been to build a relationship of cooperation between clients and health care workers. Educating the men as well as examining their fears and anxieties is useful in removing barriers of resistance and gaining trust, thus empowering the client (McEwen, 2011). The Health Belief Model (HBM) â€Å"postulates a person’s perception of disease threat and benefits of taking action will predetermine taking action† (Rivers, 2009). The HBM is used to determine the African American male understanding of prostate cancer and to identify factors that would motivate that populatio n to seek screening and treatment of prostate cancer. The HBM has been used to design intervention to educate as well as facilitate access to screening and treatment (Rivers, 2009). Leininger’s Theory of cultural care diversity is a middle range theory that is based upon the major concepts of culture, culture care and understanding the differences as well as the similarities to transcultural human care. Her other major concepts of transcultural care are, caring, and ethic views, the system of health care, the professional system of care and culturally congruent nursing care. (McEwen, 2011) The strength of Leininger Theory lies in the fact that it is a holistic theory seeking to cover both the diversity and universality of nursing care concepts. Caring aspects within and between cultures must include the acknowledgement of the similarities and differences. The theory has been widely used within the credibility and confirm ability of data gained from research (McGee, 1994). Because the Leininger Theory places emphasis on cultural s ensitivity and cultural congruence, it can be said that cultural knowledge will improve care. However, because the approach fails to take into account a political and structural inequality of minorities and focuses on differences and deficits, it can be construed that the culture itself is the problem. The possibility of reinforcing stereotypes and increasing the gap between provider and patient may actually make the care aspect less than culturally safe (Orià ¡, 2005). Leininger Theory can be used in a wide range of modalities when considering the problem of prostate screening. When examining the focus of this clinical problem i.e., how to get increased participation from a high risk group to prostate cancer screening, Plowden reached several conclusions that are applicable to today’s practice for advance nurse practitioners. For example, African-American men were more likely to seek screening and participate in the treatment process if 1.) There was a perceived susceptibility to prostate cancer and that threat was perceived as severe in life threatening or disabling . 2.) There were perceived benefits to the screening such as early detection and accurate diagnosis. 3.) There were no perceived barriers. 4.) Sufficient triggers need to exist to motivate behavior (Plowden, 2009). When applying these findings to advanced nurse practitioners’ practices, it can be said that men in the high risk groups must first be educated on the risks of prostate cancer. They need to understand the implications involved with failure to be screened. Early detection and accurate diagnosis are very important to the 5 year survival post diagnosis. Screening is essential to the quality of life for a person diagnosed with prostate cancer. For example, lifestyle changes would have to be made after a radical prostectomy versus a chemo radiation treatment of a localized slow growing tumor. When health care professionals can successfully manage to remove or go around barriers, whether real or perceived, the goal of seeking medical intervention is improved. Health care professionals can attempt to established sufficient triggers so that the behavior can be motivated to attain compliance to screening. The advanced nurse practitioner’s challenge will be to first establish trust and reputation as a caregiver who cares and is approachable; educate the population of the high risk group to the benefits of early detection versus late discovery; provide easy access to screening and the follow up phase of the intervention; and maintain confidentiality. Peer group discussions with professionals have shown to aid in getting better compliance (Plowden, 2009). The African-American males’ decision making process when considering prostate cancer screening is influenced greatly by cultural mores and beliefs. These cultural beliefs often lead to a lower rate of compliance to free screening versus all other races i.e. Caucasians, etc. Leisiniger’s cultural diversity theory can be used to effectively educate and increase compliance by teaching practitioners ways to overcome these cultural barriers. Genetic screening is a process in which DNA is tested to check if a person has any genetic indicators of diseases. It can also be defined as an experimental technique used for identification and selection of individual who possess phenotype of our interest in a mutangenized population. Thus genetic screening can also be called as phenotypic screening. It helps us to provide a better insight of functioning of gene in an individual. It has got applications in various genome project. The functioning of gene can be well determined by Forward genetic screen or Reverse genetic screen. Forward genetic screening first approaches towards the phenotype and then moves towards identifying genes i.e, genes responsible for a particular phenotype in an individual is identified. Whereas in reverse genetic screen it starts with gene or set of genes and moves towards analyzing the resultant phenotype by assaying its effect of disruption. A defined genetic background of organism being used and constant experimental procedure for identification of mutants of interest are the two key component of successful forward genetic screening. Define genetic background helps to identify and locate affected genes in mutant individual with greater efficiency (Herman .R.K, et al.,2005). Many a times genetic screening and genetic testing are considered to be a common terms, however there is difference between genetic screening and testing. Genetic screening is a technique in which set of diagnostics test are used on large number of individuals to test these individuals who are at high risk of genetic disorders and are the carriers of that specific genes, whereas genetic testing uses different laboratory test to identify the genetic status of individual. The individuals who are already suspected to be at a higher risks for a particular genetic disorder undergo the genetic testing. It is based on medical history of family or on screening tests. But the similarity between both the test is both of them undergo laboratory tests to identify the presence of specific genes which may cause genetic disorders. Genetic screening is a term that is also encountered in prenatal screening with pregnant women. Many researcher do screening on a broad spectrum in large number of people to find potential for genetic diseases. Nowadays genetic screening is also used by health insurance companies to screen the people for genetic disorders and avoid paying extra money. Genetic screening also include newborn genetic screening to identify new born who would benefit from early treatment. Reproductive genetic screening is also done to help reproductive decision making, and in this family history of the patient is checked to identify individuals who would get added benefit from additional prevention measure. Types of Genetic Screening : Presymptomatic screening Carrier screening parental screening, newborn screening, carrier screening, forensic screening and susceptibility screening. Presymptomatic screening is used to screen the patients whose health is in danger. Carrier screening is used to carry out the analysis of individuals with a gene or a chromosome abnormality that may cause problems either for offspring or the person screened.it is carried out in healthy individuals where there can be a risk of genes harmful to offsprings or future generation. This can be done by testing of blood or tissue samples and can show the presence of a particular genetic trait, changes in chromosomes, or changes in DNA that are associated with inherited diseases in asymptomatic individuals. For example carrier screening is done for sickle cell anemia, Tay-Sachs disease, duchenne muscular dystrophy, hemophilia, Huntingtons disease, and neurofibromatosis. Prenatal screening is carried out in a fetus when it is at risk for various identifiable genetic diseases or traits. It began in 1966. Newborn screening is related with the analysis of blood or tissue samples taken in early infancy in order to detect genetic diseases for which early intervention can avert serious health problems or death. Newborn screening was started in 1960 with the ability to test newborns for a rare metabolic disease, phenylketonuria (PKU). Two other examples of newborn screening are the testing of African American infants for sickle cell anemia and Ashkenazic Jews for Tay-Sachs disease. Forensic screening is used in criminal investigations it is used to to discover a genetic linkage between suspects and evidence discovered in criminal investigations. As DNA of each and every person is unique, many people are reluctant to see such information become part of any national database, which might include information not only about identity but also about proclivity toward disease or behavior. Susceptibility screening is a technique which is used to screen a selected population for genetic susceptibility to environmental hazards. It helps in the identification of workers who may be susceptible to toxic substances that are found in their workplace and may cause future disabilities. It is used to confirm the diagnosis in patients with symptoms. Genetic screening also helps to advise other family members of the diagnosed patient to detect whether they also have the disease or have genetic markers for the disease even if they dont have symptoms ,and also to check whether they are carriers or are neither. It also helps in detecting when one partner is a sufferer or carrier, to test the other partner in order to advise whether their child will have the disease, be a carrier or will not have the disease. If diseases observed in both the parents, the child must inherit the disease or the genetic markers for the disease. It may happen that one parent has the disease and the other partner also has the genetic markers for it but there are no symptoms for it, therefore the child may develop the disease or alternatively may not show any symptoms during its lifetime. If one parent has the disease and the other is a carrier, in each pregnancy there is a 50:50 chance of the child inheriting the disease. In the population, screening is carried out to discover undiagnosed sufferers or those with genetic markers for the disease, to discover carriers and to aid research into the prevalence and severity of the disease and the carrier ratio. Maternal serum alpha-fetoprotein (MSAFP) screening, enhanced MSAFP, amniocentesis, chorionic villus sampling (CVS), percutaneous umbilical blood sampling (PUBS),fetal biopsy and fetal cell sorting are some of the genetic screening tests which are currently available for pregnant women. MSAFP is a blood-screening test. It is performed at the 16-18 week gestation date and it tests for spina bifida. Enhanced MSAFP is also a similar type of blood-screening test that measures levels of certain biochemical markers to test for the presence of Downs syndrome. But the only thing is that this test only has an accuracy of 60-65%. Amniocentesis is performed at the 16-18 week of gestation. Amniocentesis uses amniotic fluid to test for chromosomal abnormalities. It is also used to find biochemical abnormalities at the genetic level, it helps in detecting up to 180 genetic disorders. CVS i.e, chorionic villus sampling screening is performed at 10-12 weeks into gestation and it uses chorion tissue f or chromosomal analysis in biochemical and DNA studies. It is not widely used because this test has a drawback of correlation to produce newborns with limb abnormalities. ).Percutaneous umbilical blood sampling, PUBS is performed after week 18 . PUBS is used only as a confirmation test based on results from previous other tests. In fetal tissues are taken for DNA testing . Fetal cell sorting includes an experimental procedure in whcich blood is taken from mother and fetal cells are tested in mothers blood. Oberle,I. D. Camerino.G, 1985 carried out genetic screening for hemophilia A(classic hemophilia) with polymorphic DNA probe. They collected blood samples after informed consent from13 families and carried out coagulation and immunologic assay. In this experiment they develop a new technique to screen the patients for hemophiliaA in the families at risk for the disease. A DNA probe (St14) that detects a very polymorphic region on the human X chromosome has been shown to be closely linked to hemophilia A. They observed that there was no recombination between the St14 locus and hemophilia A in 12 families studied. The odds in favor of linkage are 4.4 x 109to 1 (lod score, 9.65). They found that there was 0 to 6.5% probability of the 95 per cent confidence interval of a recombination between St14 and hemophilia A. This informative DNA probe which causes families in risk of hemophilia A in more than 90 per cent, can be used in conjunction with classic biologic assays to identify carriers with an accuracy of 96 per cent or more. If a small risk of misclassification due to crossover between the test and the disease loci is accepted, this DNA marker should allow first-trimester prenatal diagnosis of hemophilia A. Segregation analysis with St14 may thus represent a major improvement in genetic counseling for hemophilia A. Well, there was such a nation, and it is Portugal. Portugal was the country that lead the greatest expansion of the known world in the history of the world. In only one hundred years they managed to be the first Europeans in nearly one half the globe, as well as be the first to push south. The accomplishments of the rest of Europe paled in comparison. The Portuguese exploration and expansion at this time is a huge subject. Here will be discussed only one of their voyages, that of Vasco d a Gama's voyage to India, as he was the first European to sail to there. Prince Henry the Navigator was most definitely a driving force in this venture, while the King during the voyage, Manuel, was the one who ordered it. Bartolmeu Dias was the first to round the Cape of Good Hope, in his 1487-88 voyage, proving that it could be done, and breaking ground for da Gama. Born in 1394, the Infante Enriques, known to the world as Prince Henry the Navigator, became the man who was most responsible for the maritime expansion of Portugal and of general marine science, as well, both of which led to the voyage to India.
